Dog Daycare/Boarding Information |

| What is your application process? |
|
All dogs applying for daycare
will be given a temperament test. Guardians will be required
to fill out a written application providing information
on themselves as well as their dogs. |

| What are your vaccination requirements? |
|
Rabies, Bordetella, Parvo, Distemper |

| Have you ever turned down any dogs before?
If so, for what reasons? |
|
Yes. Chronic barkers, and dogs that showed
aggression right away towards other dogs in the daycare
setting. |

| Do you have separate areas for large dogs
and small dogs? |
|
Yes. |
|
| Is your facility indoor or outdoor? |
|
Our exercise/play areas are indoor. Your
dog will be given relief breaks outdoors every 3-4 hours. |

| How many times per day do you walk the dogs?
Can we purchase additional walks or play time? |

Every 3-4 hours. |

| Do you provide dog treats? How many
times do you feed the dogs? |

Our daycare dogs will receive a treat at
Noon when the dogs have their naptime for the day. The treats
are given only with owner's permission. |

|
All day, at all times. |

| Do you provide dog toys? Can we bring our
own toys and other personal belonging? |

Yes, we provide toys. Daycare dogs are only
allowed to bring food if they their guardian's request a
feeding while at daycare. |

| Do you have a nap time for the dogs? |
|
Yes. |

| What is your cancellation policy? |
|
Our cancellation policy is 24 hours for non-holiday
times and 7 days during major holidays. |

| Where is the nearest vet? Do you have a vet
on call? |
|
Our nearest vet is in the alley behind our
facility. |
|
| What do you do in case of emergency? |
|
1. Notify the owner.
2. Take the dog to the vet on record if close enough to
our facility.
3. In case of a life threatening emergency, the dog will
be taken to the emergency vet that is located in the alley
behind our facility. |
