Daycare and/or boarding application
and agreement completed in store. Dog must complete and
pass temperament test prior to attending. |

Up to date on all shots including Rabies,
Distemper, Parvo, and Bordetella. |

| Have you ever turned down any dogs before?
If so, for what reasons? |

Yes. Too aggressive with other dogs and/or
humans. |

| Do you have separate areas for large dogs
and small dogs? |

Yes. |

| Is your facility indoor or outdoor? |
|
Indoors. |

| How many times per day do you walk the dogs?
Can we purchase additional walks or play time? |
|
All dogs are walked every other hour and
are played with by our staff during normal business hours.
You cannot purchase more walking or playtime. |

| Do you provide dog treats? How many
times do you feed the dogs? |

Dogs that are overnight guests will be given
meals per the owners instructions. |

| How often do you provide water? |
|
There is always water available to all dogs. |
 |
| Do you provide dog toys? Can we bring our
own toys and other personal belonging? |
|
You can bring your dogs personal belongings
only if they are overnight guests. You do not need to bring
a cage. Daycare customers should not bring any items other
than their dog and leash. |

| Do you require a nap time for the dogs? |
|
Dogs naturally nap throughout the day based
on the amount of running and playing they do. Every dog
is different. |

| What is your cancellation policy? |
|
If you requested an overnight stay, we ask
that you call to cancel at least 24 hours in advance so
other customers can use our services. |

| Where is the nearest vet? Do you have a vet
on call? |
|
Unless otherwise noted, we take the dog to
their own Vet if there is an emergency. If the emergency
does not allow enough time to reach the dogs own Vet, there
is a Vet Hospital on our street. |
 |
| What do you do in case of emergency? |
|
Each situation is different and is evaluated
by the Mgr or Owner. If it is not serious, it is handled
in house. If it requires medical treatment, the dog is taken
to the Vet. |
